From Gamble to Science: How to Achieve 100% Accuracy in Predicting Drug Repurposing Success
For decades, drug repurposing has been the great promise of pharmaceutical development as a faster, cheaper, and safer path to new therapies. Yet in practice, it has remained a high-stakes gamble, often driven by serendipitous luck, limited data, and disconnected evidence.

At Basil Systems, we set out to answer that question. We developed a new methodology that integrates the most powerful — yet typically siloed — data assets in pharma: global labels, submissions, clinical studies, pharmacovigilance, and other data.
We then conducted a landmark retrospective study, applying our integrated AI platform to off-patent drugs to see if we could identify their successful label expansions before they happened, using only the data available at the time.
The results were a breakthrough. Our integrated approach delivered:
- 100% accuracy in identifying the approved new indication within the top 3 candidates
- A 4x improvement in predictive accuracy compared to traditional, siloed pharmacovigilance analysis (100% vs. 24%)
This isn’t a coincidence; it’s the result of a systematic framework that triangulates evidence to find real signals in the noise. By harmonizing these disparate datasets, we built a reproducible engine for identifying high-confidence therapeutic opportunities, effectively de-risking the entire repurposing lifecycle.
